You will receive a $5.00 Gift Certificate via email for your participation. Only one Gift Certificate will be issued per household per month. All fields must be completed to qualify. Each submission must include a valid transaction number to qualify. Only one Gift Certificate will be issued per household per month and survey must be completed with in 30 days of purchase. Friends and family members of employees are not eligible to participate in the survey. The WildSide - Tropical Fish & Exotic Pets reserves the right to limit the number of Gift Certificates issued to an individual or household. Survey answers may be used in Employee Training materials in an effort to provide excellent service to our customers.
